How do I notify Colorado Health Networks when I have a change in practice
information? A: You will need the Provider Change/Update form and a W-9. This form must be used to notify CHN of a variety of changes to your practice, such as those mentioned above. A new W-9 must be completed when there are any changes to the tax ID information, even an address change. Q: It seems as though I recently completed a recredentialing application. Why did I get another? A: We perform the recredentialing process for practitioners every 36 months, as mandated by NCQA. The process begins 30 months after your last Credentialing Committee approval to allow ample time for obtaining missing information and performing necessary verifications. Q: Nothing has changed since my last application, why do I need to take the time to complete the same information? A: Colorado Health Networks and ValueOptions are very much aware of the time expended to complete this paperwork. We are looking at a process that would allow us to pre-populate the application and you would need to make updates only to those areas where changes have occurred. Until this is available we must ask you to complete all sections. Q: I can't locate my Provider Manual. How do I obtain another?
