CCAR Gets a Makeover

Starting July 1, 2006 the State of Colorado will be using a new CCAR form. Please do NOT send CHP any of the old CCARs after 7/7/06. They will be returned to you as the state will not accept them. Completion of the CCAR is mandated by the State of Colorado. The CCAR should be completed for a client when:

  • OPENING OF CASES: Admission
  • CLOSING OF CASES: Discharge
  • UPDATING OF CASES
  • ANNUALLY: For clients in treatment for 12 months

Failure to submit a CCAR will result in the non-payment of claims.

CCAR Requirements

Completion and submission of the CCAR within the defined parameters continues to be a requirement for every provider who delivers services to Colorado Health Networks members.  A CCAR must be completed at admission to treatment and at discharge or annually, depending on which occurs first. Then, the completed CCARs must be submitted to Colorado Health Networks. The Colorado Health Networks staff realizes that the completion of this instrument may be of questionable value to some providers since provider-specific feedback has not been provided by the State. However, the data is used by the State for a variety of purposes. Furthermore, the State has the option of instituting financial penalties against Colorado Health Networks if the CCAR data is not submitted. If we don’t get paid, then you don’t get paid!

Several measures have been implemented to assist providers in the completion and submission of the CCARs:

  • The instrument has been produced in scannable form to reduce rework resulting from errors;
  • The instrument is on the web at www.chnpartnerships.com
  • We offer an honorarium of $5.00 for each completed set received.

The completed admission CCAR should be submitted to Colorado Health Networks within five business days of intake. The discharge (or annual update) CCAR should be submitted within thirty days of discharge (or the annual date of treatment beginning). The discharge CCAR is required regardless of the circumstances surrounding the discharge. For example, if an admission CCAR has been completed and the client then decides to discontinue treatment after one session either by verbally informing the provider or by failing to keep the scheduled appointment, a discharge CCAR is to be completed and submitted within thirty days of the client’s decision to stop treatment.

CCAR GETS A MAKEOVER

The Colorado Client Assessment Record (CCAR) has been revised. The updated version will make your life easier by allowing you to complete and submit electronically.  CHN’s Application Programmer, Paul Hann, performed the makeover by adding check boxes and pop-up screens to show you required fields and errors.   The form has completed the testing phase and is ready for your use.  We encourage you to access the form on-line at our web site, www.chnpartnerships.com. 

 

For those provider’s who do not have Internet access, we have hard copy forms available by contacting our Member Provider Services at 800-804-5008.   Unfortunately, we will not be able to accept copies of the new CCAR forms.  Copied forms change the parameters of the data fields and will not scan into our system.  The same problem exists with faxed forms. We recommend you mail the forms (do not fold or staple) to our office at 7150 Campus Drive, Suite 300, Colorado Springs, CO  80920. 

All required questions are marked on the paper form. If the required questions are missing or they have an invalid answer they will be returned to you for correction. The online forms will help you catch any missing or invalid answers to required questions and they will not be returned to you.
